A physical is required to participate in Volleyball and Mountain Orthopaedics is offering all Viewmont Student/Athletes free physicals on Friday August 5 from 6 am-7:30. Take advantage of this great opportunity! Here are all the details:
Important information for student/athletes needing a physical to participate in athletics at Viewmont High School:
On Friday, August 5 from 6am-7:30am the physicians at Mountain Orthopaedics, along with the physical therapists and their staff from Performance West Physical Therapy will be providing athletic physicals free of charge at their offices at the Renaissance Towne Center (1551 S. Renaissance Towne Drive, Suite 400, Bountiful, Utah) which is on the 4th floor of the building.
These physicals are for athletes that plan on participating in a sport at Viewmont High School during the 2011-12 school year and either 1) need to have their initial physical (A Form) or 2) need to have a second physical because they have had an injury since their initial physical (B Form).
If you would like to participate, please do the following:
1- Contact Jeff Emery (athletic director) - jemery@dsdmail.net and let him know that you will be participating on that day and leave your name, grade, and sport(s) on his email.
2- Go to the UHSAA website (www.uhsaa.org/new/) and you will find both the A Form and B Form there under Publications and Policies - Forms. Please print and fill out completely before you arrive for your physical on Friday, August 5. These are new forms and must and have not been updated yet on the Viewmont Website.
Copies of the physical forms will also be available in the Viewmont Business Office, online at the VHS website,and at Mountain Orthopaedics office.
3- On Friday, August 5 you need to arrive at the doctor's offices no later than 6:30am so that they can get all of the physicals completed before 7:30am. It will be first come, first serve.
